Case Management is where we work intensively with you to sort out your problems, and get you on the path to a fulfilling life.
Many of our clients are faced with daily problems, from homelessness, to ongoing issues with Centrelink or Housing, to problems with CYPS or with Pharmacotherapy providers (doctors and pharmacists). If these or other things are getting on top of you, come and speak to us about getting a case worker who can help you to sort things out.
We can help you to prioritise what's most important and to start with the most pressing issues and work towards fixing everything in your life that you're not happy with. That's not saying we can fix everything just like that, but we can help you on the path to getting there.
We can help you get to your appointments (and can even go with you if you feel the need of support).
We can help to sort out disagreements and other issues which have arisen between you and other services.
We can help with getting your I.D. sorted out, getting bank accounts set up, and we can help you to work out what's really important to you and work out how you might achieve your goals.
