We believe that our community are not cases to manage, but rather, peers who need support. CAHMA Peer Treatment Support (PTS) workers are peers who are skilled in complex case management. PTS workers will work to support your long-term health and wellbeing by walking with you, through your journey. PTS workers can help with your specific issues around drug and alcohol as well as general health, wellbeing and social support. We work to meet your individual goals and needs through person centered approaches. PTS workers are here to provide:
Carefully planning out your priorities for the next 3-6 months with a case manager
Assistance in working out what your treatment goals are and what services are available to help.
Help lining up withdrawal and rehabilitation services in a timely manner.
To friendly services which can help with a range of your social and health needs.
We can attend appointments with you to ensure that you get the best outcome and treatment choice.
In person or via phone, to help track your health and wellbeing journey.
If you are having difficulty navigating the healthcare system, we advocate for you in appointments with service providers.
With no appointments necessary, drop in for a chat with any one of our case managers and receive immediate support.
